In this case, a payment of £35.75 will be due on 1 December 2006, followed by £21.45 on the 1st of each month starting January 2007 (excluding June and July of each year).Who is eligible?Anyone in full time education between the ages of 5-15 (inclusive), and for 16 to 18 year olds staying in full time education, who or whose parents pay council tax to one of the 7 Metropolitan Borough Councils within the West Midlands County (Birmingham, Coventry, Dudley, Sandwell, Solihull, Walsall or Wolverhampton).There is no need to apply for a separate Centro 16-18 Photocard. We will issue you with one when your direct debit payment is accepted - subject to confirmation of eligibility.How do I apply?Easy! Simply click here to download an application form (PDF)Complete the form and return it to WM Card Systems, PO Box 3532, Birmingham, B9 9BG along with a colour passport sized photograph, on a light background please.
